List of documents for cadets

To register for the cycle of thematic improvement (TU) it is necessary to have the following documents:       

• pass;
• extract from the order from the place of work on a business trip;
• a copy of the employment record book;
• a copy of the diploma of graduation;
• a copy of the certificate of specialization (internship);
• copy of passport and TIN;
• a copy of the marriage certificate (for women in case of change of surname);
• medical gown.

The following documents are required for registration for secondary specialization and internship cycles:       

• pass;
• extract from the order from the place of work on a business trip;
• a copy of the employment record book;
• a copy of the diploma of graduation;
• a copy of the certificate of specialization (internship);
• copy of passport and TIN;
• a copy of the marriage certificate (for women in case of change of surname);
• photo (3 * 4cm);
• medical gown.
